McMan is recruiting for an Evening Case Manager to join our team providing a housing first and harm reduction approach; the Case Manager will be responsible for assisting residents in maintaining their housing, assist accessing financial supports, supporting their education/employment goals and any other areas of support that is required to help them successfully transition into independence. The Case Manager will work alongside our Transition Specialists, by being a member of the resident’s team that is planning around the needs for their successful transition into the community.
Some of the key aspects of this role include:
- Complete case management responsibilities that include assessments, goal planning, case consultation, monitoring and evaluation of the goals to meet the client’s needs.
- Educate and support individuals to increase skills that promote greater self-efficacy.
- Provide individual instruction, modeling, and support of basic life skills (ie: health, nutrition, home cleanliness, safety).
- Monitor and support individuals through a harm reduction approach to ensure living conditions are safe and sanitary.
- Implement integrated service delivery from the point of referral to program completion and transition for individuals accessing the program.
The successful candidate will have:
- A degree or diploma in Human Services and 3 years direct work experience
- Understanding of Rapid Rehousing Model preferred
- Demonstrated interest and knowledge of the Homeless Serving System of Care
- Strong writing, organizational, program planning and interpersonal skills
- Demonstrated interest and knowledge of Indigenous history, language, and culture
- Experience with HMIS data preferred
- Current standard First Aid Certification
- A current (no more than 6 months old) Police/Vulnerable Sector Check and Child Intervention check
- Have a reliable vehicle and proof of adequate vehicle insurance
- This role requires flexibility in work hours for early evenings due to trainings and team meetings
*This role will require on call support on a rotating basis and some evening and weekend work hours.

Salary Range: $49,768.00 - $54,931.00 annually
